Domestic Violence Coordinator - Full-time - DCMC
The Domestic Violence Coordinator provides trauma-informed psychosocial assessment, intervention, and care coordination for patients experiencing domestic violence and related psychosocial challenges. This role collaborates closely with clinical staff and community partners to support patient safety, recovery, and access to resources, while ensuring compliance with reporting and grant requirements.
Position Objective
In accordance with established department and professional guidelines, provides comprehensive psychosocial assessments for patients.
Essential Job Duties
1. Responds to patient/family needs related to abuse, domestic violence, trauma, mental health conditions, substance use, and other complex psychosocial issues using a therapeutic framework for effective problem solving with patients and family.
2. Conducts comprehensive psychosocial assessments and collaborates with nursing and interdisciplinary care teams to develop and implement individualized plan of care.
3. Provides patients with information about safety, legal options, and community resources.
4. Coordinates complex discharge planning for patients experiencing domestic violence, homelessness, substance use disorders, psychiatric conditions, or dual diagnoses.
5. Identifies, facilitates, and monitors referrals to community agencies and support services in collaboration with internal and external stakeholders.
6. Ensures timely completion of mandatory reporting and documentation requirements in compliance with organization policies, state regulations and grant guidelines.
7. Collects data, statistics, and outcomes required for grant reporting and program evaluation.
8. Documents all patient assessments, interventions, and follow-up activities in the medical record according to departmental standards.
9. Provide education and training to clinical staff on recognizing signs of abuse and domestic violence and navigating the referral process.
10. Conducts follow-up consultations with patients to support continuity of care and ongoing safety planning.
Educational/Experience Requirements
· Bachelor’s degree (BS/BA) degree in social work, psychology, nursing, or related field
or
· Four (4) years’ experience in Domestic Violence or abuse counseling with a minimum of two (2) years of completed college coursework. One year of experience working with victims of abuse is required.
Preferred (if applicable):
· Experience in healthcare, emergency, or hospital‑based settings
· Knowledge of trauma‑informed care principles
· Experience with grant‑funded programs and reporting requirements
