Position Objective:

The Program Coordinator supports the operational, patient navigation, and administrative functions of the Breast and Cervical Cancer Program (BCCP). This role integrates patient intake and navigation, clinic workflow coordination, data management, fiscal tracking, and programmatic support to ensure high-quality, compliant, and patient-centered screening and diagnostic services.

Working in close collaboration with the Medical Case Manager, the Program Coordinator ensures timely access to breast and cervical cancer screening, diagnostic follow-up, and navigation services for funded and non-funded clients. The Program Coordinator serves as one of the primary contacts for the BCCP program and supports overall program performance, compliance, and patient engagement goals.

Essential Functions:

1.    Conduct patient intake, eligibility screening, and navigation for breast and cervical cancer screening services.

2.    Schedule screening appointments and coordinate diagnostic follow-up care.

3.    Identify and address barriers to care (e.g., transportation, insurance, scheduling, follow-up compliance).

4.    Provide patient education on screening procedures, diagnostics, and available program services.

5.    Perform outreach, recalls, and follow-ups to ensure timely care and continuity.

6.    Collaborate with the Medical Case Manager to review results, determine next steps, and ensure appropriate documentation.

7.    Coordinate care with providers, imaging centers, and specialty offices.

8.    Serve as a key point of contact for program participants and partners.

9.    Maintain accurate data entry, tracking, and reporting in compliance with state and grant requirements.

10.  Support quality assurance, audits, and performance reporting.

11.  Assist with billing, budget tracking, contracts, and inventory management.

12.  Participate in community outreach, health events, and recruitment efforts to expand program reach.

Educational/Experience Requirements:

  • Experience in public health, health administration, medical assisting, case management, or related field preferred.
  • 2–5 years of experience in medical assisting, case management, patient navigation, public health programs, or clinic operations.
  • Familiarity with electronic health records and data tracking systems preferred.
  • Experience with eligibility determination, insurance coordination, or grant-funded programs strongly preferred.
  • Strong organizational and time-management skills with the ability to manage multiple priorities in a fast-paced environment.
  • Demonstrated ability to work collaboratively within interdisciplinary healthcare teams.
  • Commitment to culturally responsive care and advancing health equity.
  • Bilingual is a plus.

$24$32 USD
