Maternal Health Community Health Worker
Title: Maternal Health Community Health Worker (CHW)
Department: Community Health Improvement
Location: Community-based / Hybrid (Anne Arundel Medical Center & Doctors Community Medical Center)
Reports To: Director, Community Health Improvement
Position Objective
The Maternal Health Community Health Worker (CHW) focuses on improving maternal and infant health outcomes by providing culturally appropriate education, advocacy, care coordination, and resource navigation. The CHW supports clients during the preconception workshops by addressing social determinants of health and promoting health equity within the community.
Key Responsibilities
Client Support & Care Coordination
- Conduct outreach to expecting mothers who might participate in the preconception program
- Assist the community maternal health nurse in facilitating group sessions and workshops
Navigation & Resource Connection
- Screen clients for social needs (housing, food, insurance, employment, domestic violence, etc.)
- Connect families to community resources and social services
Advocacy & Cultural Brokerage
- Advocate for clients within healthcare and social service systems
- Address barriers related to language, culture, and health literacy
Data Collection & Documentation
- Maintain accurate client records and progress notes
- Track program outcomes and referrals
- Participate in program evaluation and quality improvement
Community Engagement
- Build relationships with community partners and organizations
- Participate in outreach events, health fairs, and community meetings
- Support maternal health initiatives and campaigns
Qualifications
Required
- High school diploma or GED (Associate’s or Bachelor’s preferred)
- Lived experience in the community served preferred
- Strong interpersonal and communication skills
- Knowledge of maternal and infant health
- Ability to build trust and rapport with diverse populations
- Basic computer skills (EHR, Microsoft Office, data entry)
- Reliable transportation and willingness to travel locally
Preferred
- CHW certification (or willingness to obtain)
- Experience working with:
- Pregnant/postpartum populations
- Migrant communities, women, and children
- Bilingual (preferred but not required)
- Knowledge of local community resources
Work Environment & Physical Requirements:
- This position involves frequent travel within the community
- Occasional evening and weekend work may be required to accommodate patient needs and community events.
- Ability to lift and carry equipment and supplies up to 25 pounds.
- Capability to perform physical activities such as walking, standing, and sitting for extended periods
