Job Title: Maternal Health Community Health Nurse

Department: Community Health Improvement

Location: Community-based / Hybrid (Anne Arundel Medical Center & Doctors Community Medical Center)

Reports To: Director Community Health Improvement

Position Objective:

The Maternal Health Community Health Nurse is responsible for providing preconception health education to women, expectant, and new mothers within the community. This role involves development of the preconception program, facilitating group classes, developing educational materials, conducting screenings, connecting patients to services, and collaborating with other healthcare providers to ensure optimal maternal health outcomes.

Essential Duties:

  • Health Education and Counseling:
    • Facilitate group classes on preconception education.
    • Develop maternal health education materials
    • Provide education and support for mothers experiencing health or social challenges.
    • Advocate for healthy lifestyle choices and preventive care measures.
  • Collaboration and Referral:
    • Collaborate with healthcare providers, social workers, and community organizations to ensure comprehensive care.
    • Refer mothers and families to appropriate healthcare services and community resources as needed.
    • Participate in case management meetings and multidisciplinary teams discuss and plan patient care.
  • Documentation and Reporting:
    • Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes.
    • Report any significant health concerns or changes in patient status to the appropriate healthcare provider.
    • Collect and report data on maternal and infant health outcomes to support program evaluation and improvement.
  • Health Assessment and Screening:
    • Comprehensive health assessments to identify pre-existing medical conditions, lifestyle factors, and potential risk factors.
    • Screening for chronic diseases (e.g., diabetes, hypertension), infections (e.g., STIs), and genetic conditions.

Qualifications:

  • Education:
    • Bachelor's Degree in Nursing (BSN) from an accredited institution.
    • Registered Nurse (RN) license in the state of practice.
  • Experience:
    • Minimum of 3 years of nursing experience, preferably in maternal and child health or community health settings.
    • Experience in preconception education classes
    • Strong clinical assessment and critical thinking skills.
    • Excellent communication and interpersonal skills.
    • Ability to educate and counsel patients and families effectively.
    • Proficiency in maintaining accurate medical records and documentation.
    • Knowledge of community resources and healthcare systems.
    • Bilingual (preferred but not required)

Work Environment & Physical Requirements:

  • This position involves frequent travel within the community
  • Occasional evening and weekend work may be required to accommodate patient needs and community events.
  • Ability to lift and carry equipment and supplies up to 25 pounds.
  • Capability to perform physical activities such as walking, standing, and sitting for extended periods

 

Pay Range
$80,000$120,000 USD
