Registered Nurse - Clinical Triage (onsite role)

Full Time - Day shift (M-F, 8-4:30pm, 40 hrs, no weekends or holidays)

Department:  LH Women’s Collaborative Care (ambulatory practice)  

Reports To:  Practice Manager

FLSA Status:  Non-Exempt

Position Objective:

The Clinical Triage RN is responsible for providing triage for all incoming medical advice questions via telephone or MyChart to ensure prompt identification of patients with high-risk conditions, assessing the need for clinical referral, coordinating care and providing education.  In addition, this position will make outbound phones calls to provide patient results for lab work and imaging.  The Clinical Triage RN is a key member of the care team working to provide quality comprehensive services to patients to include assisting with in-office procedures and managing patients enrolled in the remote blood pressure monitoring program.  

Essential Job Duties:

  1. Accurately documents symptoms/complaints, clinical assessment, advice provided and patient/caller response
  2. Utilize standing orders, triage protocols and algorithms appropriately
  3. Manage prescription refills per protocol
  4. Provide patient education
  5. Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation and follow-up
  6. Obtains appropriate input and direction from the providers as needed to determine best clinical course of action and recommendations for specific patient health care needs
  7. Coordinates and schedules same day appointments based on clinical need
  8. Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines
  9. Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care
  10. Assist with in-office procedures and cross cover as needed within the Women’s Health practices

Educational/Experience Requirements:

  • Minimum of 2 years’ OB-Gyn nursing experience preferred
  • Excellent customer service skills required.  Must demonstrate excellent professional and courteous interaction with patients, co-workers, physicians, and other health care professionals. 
  • Excellent verbal and written communication skills
  • Bilingual highly preferred  

Required License/Certifications:

  • Registered Nurse license issued by Maryland Board of Nursing or Compact State
  • American Heart Association HealthCare Provider BLS

Working Conditions, Equipment, Physical Demands:

There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.

Physical Demands:

Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
