Position Objective:

 

The Quality Coordinator- MDPCP is responsible for managing the quality metric data collection and entry as displayed in the EMR (Electronic Medical Record) and/or payor portals. that reflectpractice performance for organizational quality aims and/or incentive programs. Data management could require medical record research, communication with laboratories, diagnostic imaging centers, specialists, and/or patient outreach.

The MDPCP Coordinator performs a variety of administrative and clinical tasks to help improve care to patients, and support providers. They are an essential part of the advanced primary care team and facilitate meeting the goals of the Maryland Primary Care Program and Value-Based Contracts. The main responsibility of the role is to validate data, report, coordinate staffing, new employee orientation/training, oversite, and/or audit the day-to-day workflow of the Advanced Medical Assistant to ensure the accuracy of data.

  1. Run reports and distributes them to the AMA team on a timely basis, interpret panel metric reports to identify deficiencies, and review electronic medical records for missing or incomplete documentation.
  2. Accurate, complete, and timely data input within the bounds of record access and in compliance with the processes and procedures governing the electronic health record, CRISP, and/or payor portal(s).
  3. Accurate and timely reporting of metrics to Program Leadership.
  4. Demonstrates knowledge of quality incentive metrics – Hypertension, Diabetes, Colorectal screening, transitional care management, Diabetic Assessment completion and gaps in care, and Population Health organizational goals.
  5. Reviews the work of the AMA staff as instructed by Senior Manager of Value-Based Programs and conducts quarterly audits. 
  6. Oversee and train new and existing AMA at the direction of the Program Leadership.
  7. Works collaboratively with the Care Management Team to improve patient outcomes and assist in organizing and participating in quality improvement activities using the PDSA model to improve systems and services.
  8. Perform other administrative tasks and clinical care as directed by the Senior Manager of the Value-Based Program
  9. Assist with daily and monthly quality assurance reviews including TCM/ ED tracking, primary care referral tracking,and other programs as assigned
  10. Reports performance concerns and plans to Senior Manager of Value-Based Program. 
  11. Performs other related projects and duties as assigned.

 

 

Educational/Experience Requirements:

  • Minimum High School Diploma
  • Minimum of three (3) years of experience as a Medical Assistant, demonstrated experience in a lead role as an Advanced Medical Assistant.
  • Experience with Microsoft Office
  • Experience operating phone systems, fax machines, copiers, and PC’s

 

Required License/Certifications: 

  • CMA, RMA or AAMA (through an approved/accredited organization). Must maintain Certification or credentials while employed

Pay Range
$17.30$23.20 USD
