Patient Access Clinical Triage Representative-Patient Access and Support Services
Position Objective:
The Patient Access Clinical Triage Rep is responsible for providing triage (sorting/prioritizing patients) telephone service to ensure prompt identification of patient with high-risk conditions, assessing the need for clinical referral, providing education, and carrying out such other Medical Assistant duties as PASS may need. The Patient Access Clinical Triage Rep is a key member of the care team working to provide quality comprehensive services to patients while collaboratively with staff to provide team-based coordinated care. The Patient Access Clinical Triage Rep will demonstrate the values consistent with Patient Access and Support Service’s mission.
Essential Job Duties:

- Accurately documents symptoms/complaints, clinical assessment, advice provided and patient/caller response.
- Utilize standing orders, triage protocols and algorithms appropriately
- Manage prescription refills per protocol
- Provide patient education: e.g. Chronic disease, preventative and family
- Participates in Performance Improvement activities as assigned
- Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation and follow-up
- Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
- Coordinates and schedules same day appointments based on clinical need
- Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
- Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.
Educational/Experience Requirements:
- Three to five years working in an Ambulatory Care or related field. Demonstrated experience in staff development theory and/or principles of adult learning
- Excellent customer service skills required. Must demonstrate excellent professional and courteous interaction with patients, co-workers, physicians, and other health care professionals.
- Strong technical skills, preferably using an Electronic Health Record software
- Excellent verbal and written communication skills
Required License/Certifications:
- Registered or Certified Medical Assistant
- CPR Certification
Working Conditions, Equipment, Physical Demands:
There is a reasonable expectation that employees in this position will not be exposed to blood-borne pathogens.
Physical Demands:
Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
