Position Objective:
The Manager Value-Based Programs will oversee the day-to-day operations and strategic value-based care initiatives. This role drives high-quality performance, care gap closures and low-cost outcomes by developing and implementing best practices, policies, and/or procedures. The Senior Manager Value-Based Programs is responsible for ensuring program employees have the tools needed to achieve quality outcomes and organizational goals. The position engages practice leadership to execute specific, measurable goals, including access, adherence to evidence-based medicine, and discipline-specific outcome measures. They play an integral role in promoting access to appropriate care, including overseeing care management and coordination activities for the patients of the practice.
Essential Job Duties:

- Select, coach, and lead all certified and/or licensed program employees.
- Processes bi-weekly payroll and grant-funded expenses for all program employees.
- Meet with program and office staff at regular intervals to implement new programs, and procedures, monitor personnel issues, and improve operational efficiencies.
- Provide operational team leadership, training, and guidance regarding program management, data analytics, and performance improvement initiatives.
- Communicates quality metrics and program requirements to employees.
- Implement and monitor key performance metrics. Report out to Director and Senior Leadership.
- Manage administrative requirements of programs to include reports, budget oversite, and audit trail.
- Develop, implement, and monitor quality improvement activities in alignment with organizational and service line goals.
- Develop and implement best practice workflows (quality assurance/control) to ensure optimal delivery of care: to include workflow, staff training, practice site visits and audits for compliance.
- Develop and maintain effective working relationships with internal and external stakeholders to support the success of population health initiatives.
- Grant Liaison: pursue funding opportunities to enhance and/or expand existing population health resources. Responsible for all aspects of grant management.
- Participates in conitnuous evaluation of program effectiveness, identifying opportunities for improvement.
Educational/Experience Requirements:
- Strong presentation skills. Motivated, achievement-oriented, and able to multitask.
- Preferred: Associate’s degree or Bachelor’s degree in business administration, health administration, finance or related field highly desired or, equivalent in practice management experience.
- Minimum of 5 years working in a clinical setting, 3+ years of proven success in leadership roles;
- Excellent computer skills and familiarity with Microsoft Office programs;
- Superior interpersonal communication
- Strong organizational and time management skills;
- Demonstrate an understanding of standard clinical procedures, laws, and regulations;
- Thorough knowledge of medical terminology;
