Diabetes Instructor - Lead
Position Objective:
The Luminis Health Anne Arundel Medical Center Diabetes Center is a recognized diabetes self-management education (DSMES) program by the American Diabetes Association. Assists Medical Director with administrative duties, such as quality assurance, staffing and operations, promotion, and program growth. Independently provides immediate care services in satellite settings and main office to a broad range of patients. Follows evidence-based guidelines and protocols, and documents appropriately in electronic medical record. Serves as the Diabetes Quality Coordinator (QC) and is responsible for overseeing the day-to day operations of the DSMES service at all sites. The QC ensures that the National Standards (NSDSMES) are met and maintained at all times.

- Supports the vision, mission, and goals of the medical center through behaviors that foster professionalism, empowerment, teamwork, productivity, and customer satisfaction.
- Assists medical director in the development and adoption of evidence-based clinical protocols.
- Monitors patient and community practice satisfaction. Assists medical director in the development of performance measures, incentives and continuing education programs for all Certified Diabetes Care and Education Specialists. Along with the medical director, monitors the performance of all staff in adhering to standards of care delivery, professionalism, patient privacy and risk management.
- Ensures adequate staffing and supplies for all sites at all times. Monitors staff adherence to corporate policy and procedures. Ensures staff have appropriate on-boarding and training to perform to standards. Assists medical director with budget planning and staff recruitment.
- Functions as an expert clinical resource in diabetes management and fosters interdisciplinary collaboration by serving on hospital-based service line committees. Collects data pertinent to the patient’s health and situation including interpreting blood sugar logs and insulin pump logs relevant to the patient’s status.
- Identifies assessment, diagnostic strategies, and therapeutic interventions within the plan that reflect current evidence, including data, research, literature and expert clinical knowledge; collaborates with physician on the plan as needed.
- Promotes and utilizes customer service data for improvement of diabetes services. Ensures effective on-going team communication and information sharing at inter and intra levels of assigned areas.
- Oversees the planning, implementation and evaluation of the DSMES service. Arranges and coordinates the activities of the Advisory Group.
- Liaises between the DSMES team members, the Advisory Group, other departments and administration. Monitors and facilitates maintenance of DSMES team members’ qualification (CE credits, training, competency, licensures, and registrations).
- Responsible for maintaining American Diabetes Association Recognition and participating in the evaluation of the DSMES service’s effectiveness. Uses current health care research findings and other evidence to expand clinical knowledge, and enhance role performance and increase knowledge of professional issues.
Educational/Experience Requirements:
- Graduation from a school of nursing with licensure as a Registered Nurse or Registered Dietician by the State of Maryland.
- Prior experience in providing diabetes education, medical nutrition therapy for Type 1, Type 2 and gestational diabetes is required.
- Three years staff nursing experience with at least one year in diabetes specialty and one year demonstrated clinical leadership role.
Required License/Certifications:
- Certification as a Diabetes Educator from the American Association of Diabetes Educators or is eligible.

There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.
Physical Demands -
Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
